As a part of the 2019 Live Surgery Open Rhinoplasty Course, Dr. Robotti does six operations. Each case includes a step-by-step description of the operational approach, from marks to closure and dressing, together with clinical pictures, cone-beam CT analysis, and three-dimensional renderings. Technical tips on using power tools and piezo for osteoplasty, osteotomy, and septoplasty, as well as cartilaginous pushdown to restore or maintain beautiful and natural dorsal lines, will be helpful to viewers. Dorsal management through structural restoration will also be covered.
Four instances demonstrating the new piezo-assisted turbinoplasty are presented, along with an overview of the instruments’ utility. Three patients provide comprehensive examples of delicate rib grafting for secondary reconstruction as well as the novel Sandwich of Perichondrium, Rib Lamina, and Rectus Fascia (SPLF) approach for accurate dorsal reconstruction. The educational value of this video collection is enhanced by Dr. Robotti’s remarks and the audience Q&A.

Case 1: Main, a 38-year-old man with a prominent hump on his huge nose
Case 2: The female patient, aged 24, had a deviated nose with a hump and a septal abnormality.
Case 3: Pinched middle third and tip deformity in a 26-year-old female primary patient
Case 4: Secondary deviated nose with saddling, tip deformity/asymmetry, pinching, and alar retraction in a 33-year-old male
Case 5: A 56-year-old female secondary patient with dorsal and infratip abnormalities, boxy and pinched tip
Case 6: A 53-year-old female secondary patient with tertiary characteristics: a large middle vault, septal fistula, overshortened and overrotated tip, overresected lateral crura with pinching, and insufficiency of the external valve
